Group Clinic Registration

This form is for players currently participating in our programs

Submission of Form does not guarantee automatic enrollment. Space is limited and slots are assigned on a first come, first served basis during the sign up window.  A follow up e-mail from Plus 1 Tennis Academy will be sent confirming participation.


Before submitting your registration form, we ask that parents and players review our Player Code of Conduct and our Policies regarding Refunds, Cancellations, Credits, Schedule Changes, Drop-Ins and Make Ups.
